Early Recognition of Actinic Keratosis in an Aging Population

Healthy aging has changed dramatically over the past several decades. Preventive medicine has expanded well beyond treating disease to identifying health risks before they become serious problems. Today, patients routinely discuss blood pressure, cholesterol, diabetes, bone health, and cancer screening with their healthcare providers. They are encouraged to stay physically active, eat well, and take an active role in preserving their health as they age.

One area of prevention, however, has not kept pace with the rest of modern healthcare.

In my view, skin health remains an underappreciated part of healthy aging. We devote considerable attention to conditions that develop over many years, yet many people have never been taught to recognize one of the most common signs of chronic sun damage: actinic keratosis.

Actinic keratosis (AK) is the most common precancerous lesion of the skin and the most common precursor to cutaneous squamous cell carcinoma. Despite its prevalence, early lesions are frequently overlooked by patients because they resemble ordinary dry skin or age-related changes. According to the American Academy of Dermatology, they are often easier to feel than to see. Patients commonly describe a rough patch long before they notice a visible lesion.

That distinction is important because early recognition often begins with awareness.

Many people regularly monitor their blood pressure or cholesterol but rarely give the same attention to persistent changes in their skin. A rough patch that remains for weeks or months is easily dismissed as another inconvenience of getting older. In reality, it may represent years of accumulated ultraviolet damage that deserves medical evaluation.

The need for greater awareness is becoming increasingly important as the American population ages. By 2030, every baby boomer will be at least 65 years old. Many members of this generation spent much of their youth outdoors before sunscreen became part of everyday life. Tanning was encouraged, prolonged sun exposure was common, and the long-term consequences of ultraviolet radiation were not yet widely understood. Although our knowledge has changed, the effects of those earlier years remain.

Actinic keratosis reflects that history.

Unlike an acute sunburn, ultraviolet damage accumulates gradually. Over decades, repeated exposure produces genetic injury within the keratinocytes that make up the outer layer of the skin. Eventually those changes may appear as rough, scaly lesions on chronically sun-exposed areas such as the face, scalp, ears, neck, forearms, and backs of the hands.

Occupational exposure has also played an important role. Construction workers, farmers, roofers, landscapers, military personnel, fishermen, and others who spent years working outdoors often accumulated far greater ultraviolet exposure than the average person. As these populations grow older, clinicians are seeing the long-term effects of those decades of exposure with increasing frequency.

Current estimates suggest that between 40 and 58 million Americans have actinic keratosis. Prevalence increases steadily with age and cumulative sun exposure, exceeding 50 percent in some high-risk populations. For a condition this common—and one that represents an opportunity to intervene before invasive skin cancer develops—it receives remarkably little public attention.

Age remains the strongest risk factor because the injury is cumulative. Every year of unprotected sun exposure adds to the total burden of damage while the skin’s ability to repair itself gradually declines. Fair skin, light-colored eyes, a history of frequent sunburns, immunosuppression, and extensive occupational or recreational sun exposure further increase risk.

One aspect of early detection deserves more attention than it typically receives.

Many suspicious lesions are first noticed not by patients themselves but by spouses, adult children, or caregivers. A rough patch on the scalp, the back of an ear, or the neck may be impossible for someone to see without assistance. Encouraging family members to participate in skin awareness can become another practical tool for early recognition.

It is also important to remember that actinic keratosis is more than an isolated lesion. It is a marker of cumulative sun damage.

Patients often develop multiple lesions within areas of chronically damaged skin, a concept known as field cancerization. Even skin that appears normal may contain microscopic genetic changes resulting from years of ultraviolet exposure. Finding one lesion should therefore encourage careful evaluation of the surrounding area rather than focusing only on a single spot.

Patients should seek medical evaluation for rough or gritty patches that persist, lesions that repeatedly crust or bleed, areas that continue to enlarge or thicken, or persistent scaling of the lower lip that may represent actinic cheilitis. These findings are not diagnostic of actinic keratosis, and many benign skin conditions may appear similar. Appropriate clinical evaluation—and biopsy when indicated—remains essential.

One of the greatest successes of modern medicine has been shifting attention toward earlier recognition of disease. Screening programs have reduced illness and improved outcomes because they identify problems before complications occur. Skin health deserves to be viewed through that same preventive lens.

Routine skin assessments need not be lengthy or complicated. Incorporating them into periodic wellness examinations, while encouraging patients to become familiar with their own skin, creates another opportunity to identify suspicious lesions earlier in their development.

Primary care physicians, dermatologists, nurse practitioners, physician assistants, and other healthcare professionals all have a role in reinforcing this message. Just as patients learn to recognize warning signs of stroke or heart attack, they should also understand that a persistent rough patch on sun-exposed skin is worth discussing with a healthcare professional.

As healthcare professionals, we cannot prevent every skin cancer. We can, however, improve the likelihood that precancerous changes are recognized before they progress. Education remains one of the most effective tools we have.

Healthy aging is not simply about adding years to life. It is about preserving health across every organ system—including the one we see every day.
