The False Economy of Returning to Manual PGD Processes

Across healthcare, financial pressure is forcing difficult decisions. Pharmacy teams are being asked to do more with less. Service expansion, dealing with NHS patients, workforce shortages remain acute, and every investment is being scrutinised.

In this environment, it can be tempting to view digital systems as a cost that can be reduced or removed. Equally, there can be pressure to opt for the lowest cost digital alternatives, even if they add little value to clinical practice. Some organisations are revisiting paper based Patient Group Directions (PGDs) in an effort to cut expenditure. On the surface, the logic appears sound. If budgets are tight, surely reducing technology costs helps protect the bottom line. The reality is often far more complex.

What appears to be a saving on paper can create hidden operational costs that ripple throughout the entire healthcare system. When clinical workflows become slower, more fragmented, and more dependent on manual administration, the burden ultimately falls on healthcare professionals and patients alike.

The question healthcare leaders should be asking is not whether they can afford well designed digital tools, but whether they can afford to operate efficiently without them.

The Cost of Clinician Time

Every minute a pharmacist, clinician, or healthcare professional spends completing administrative tasks is a minute they are not spending delivering patient care. Yet administrative workload continues to grow across healthcare settings.

Manual PGD processes can require clinicians to repeatedly complete documentation, verify eligibility criteria, check records, and manage paperwork that could otherwise be streamlined through appropriately designed digital systems. The same can also be true of poorly designed digital tools, which simply replicate paper processes on a screen rather than improving them. While each individual task may only take a few minutes, the cumulative impact across hundreds or thousands of consultations becomes significant.

When viewed purely through a technology budget lens, removing a digital platform may appear to save money. However, when the additional clinician time required to complete the same work manually is factored in, the equation changes considerably.

In many cases, organisations end up spending more through reduced productivity, slower service delivery, and diminished capacity than they save through cutting technology expenditure.

Small Frictions Create Large Operational Problems

One of the biggest challenges with manual processes is that inefficiencies rarely appear as a single dramatic issue. Instead, they emerge through countless small points of friction.

A form that must be completed twice. A patient record that requires manual verification. A document that needs to be printed, stored, retrieved, or archived. A consultation that takes a few minutes longer than necessary.

Individually, these delays may seem insignificant. Collectively, they can have a substantial impact on service delivery.

For pharmacy teams already operating under pressure, even small inefficiencies can reduce the number of patients they are able to see each day. This becomes particularly important as pharmacies continue to take on an increasingly important role in delivering front-line healthcare services.

As healthcare systems seek to improve access and reduce pressure on other parts of the system, capacity matters. Every unnecessary administrative task reduces the time available for clinical care.

Consistency Matters in Clinical Practice

Manual processes inevitably introduce variation. Different individuals may record information differently, follow workflows in slightly different ways, or interpret guidance inconsistently. While healthcare professionals work diligently to maintain high standards, systems that rely heavily on manual administration naturally create more opportunities for inconsistency.

Well designed digital workflows can help establish standardised processes that support clinical decision making while maintaining appropriate professional oversight. Simply digitising a manual process isn’t enough. Technology should reduce complexity, not recreate it in another format. They can ensure information is captured consistently, documentation requirements are met, and audit trails are maintained.

This is not about replacing professional judgement. It is about providing the infrastructure that allows clinicians to focus on applying their expertise rather than navigating administrative complexity.

As healthcare becomes increasingly data-driven and regulatory expectations continue to evolve, consistency and traceability become even more important operational considerations.

The Impact on Patient Experience

Operational inefficiencies are often discussed in terms of organisational performance, but patients experience their effects directly.

Longer consultation times can mean longer waiting times. Reduced clinical capacity can limit access to services. Administrative bottlenecks can slow treatment pathways and create frustration for both patients and healthcare professionals.

Patients increasingly expect healthcare interactions to be efficient, accessible, and seamless. While clinical quality rightly remains the priority, operational excellence plays an important role in delivering a positive patient experience.

When clinicians spend less time managing paperwork and more time engaging with patients, everyone benefits.

Digital Tools Should Support, Not Replace, Healthcare Professionals

Healthcare professionals are understandably wary of technology that creates additional administrative burden, introduces complexity, or attempts to replace clinical expertise. Poorly designed systems can become part of the problem rather than the solution. If technology adds clicks, duplicates paperwork or interrupts consultations, it risks creating the same inefficiencies as manual processes, despite being digital. 

The most effective digital tools are those that work quietly in the background, reducing unnecessary administration and supporting existing clinical workflows. Their role is not to replace healthcare professionals but to enable them to practise at the top of their licence. This distinction is important.

The goal should never be digitisation for its own sake. Instead, healthcare organisations should focus on adopting technologies that demonstrably improve efficiency, consistency, and patient outcomes while respecting the realities of clinical practice.

Looking Beyond Immediate Savings

Financial sustainability remains a legitimate concern across healthcare. Every organisation must carefully evaluate spending and ensure resources are being used effectively.

However, decisions should be informed by the full operational picture rather than headline cost reductions alone. The choice is between solutions that genuinely improve clinical workflows and those that create additional burden, regardless of the format. 

When assessing the value of digital clinical workflows, organisations should consider the wider implications for productivity, workforce capacity, consistency, governance, and patient experience. What appears to be a saving in one budget line may create significantly greater costs elsewhere.

Healthcare systems face enough challenges without introducing additional administrative friction. As demand continues to rise and workforce pressures persist, supporting clinicians to work efficiently should remain a strategic priority.

The future of healthcare will undoubtedly continue to combine professional expertise with digital support. The organisations that succeed will be those that recognise technology not as an optional extra, but as an enabler of better, more efficient care.
