Vaxcyte reported positive topline results from OPUS-1 on Monday 5 October 2026. The pivotal adult Phase 3 tested VAX-31, the company’s 31-valent pneumococcal conjugate vaccine. It is the first of three adult Phase 3 trials to report. The release was datelined San Carlos, California, at 06:00 Eastern. All prespecified primary immune-response endpoints were met, and the shares rose sharply. The detail beneath that headline is more mixed. It also reads differently in Britain than in the United States.

What OPUS-1 measured

OPUS-1 randomised 4,047 participants across about 30 United States sites. The design was randomised, double-blind and active-controlled. Adults aged 50 and over, 3,572 of them, were allocated equally to VAX-31, Prevnar 20 or Capvaxive. A separate cohort of 475 adults aged 18 to 49 received VAX-31 or Prevnar 20, three to one. Immune responses were assessed one month after vaccination. Safety is assessed for six months after vaccination. Both comparators are licensed pneumococcal conjugate vaccines. Prevnar 20 is sold as Prevenar 20 in the United Kingdom.

The registry identifier is NCT07284654. Phase, enrolment and endpoints here come from the sponsor’s release.

The co-primary endpoints used opsonophagocytic activity titre ratios. Eleven serotypes are shared with both comparators, and all eleven met non-inferiority. For those eleven, the prespecified analysis required non-inferiority against at least one comparator, not both. The nine shared with Prevnar 20 alone met it too, as did the eight shared with Capvaxive alone. Three serotypes unique to VAX-31, plus cross-reactive serotype 20B, met a separate superiority criterion. Vaxcyte states that all 32 serotype comparisons in the younger cohort met non-inferiority against adults aged 50 to 64. That count covers the 31 vaccine serotypes plus cross-reactive serotype 20B.

The two serotypes that fell short

Against Prevnar 20 alone, all 20 shared serotypes cleared a lower bound above 0.667. Against Capvaxive, 17 of 19 did. That 0.667 bound is the trial’s prespecified non-inferiority criterion. Vaxcyte states that serotypes 3 and 12F “missed the LBCI >0.667 noninferiority criterion and met the historical LBCI >0.5 threshold”. Both serotypes appear in Prevnar 20 and in Capvaxive, according to the two British labels. The shortfall therefore sits against one comparator, not both.

That rule is why the co-primary endpoints were still met. Serotypes 3 and 12F matched Prevnar 20, so both counted as successes. BioPharma Dive reported analyst commentary making the same point.

Safety findings, and the limits of a topline

Vaxcyte reports that VAX-31 “was well tolerated”. It describes the safety profile as “similar to PCV20 and PCV21 across all ages studied”. Solicited local and systemic reactions from day one to day seven were generally mild to moderate. Most resolved within 48 hours. The release reports no serious adverse events considered vaccine-related, and no discontinuations because of adverse events. Both are the sponsor’s statements.

What the release omits matters as much. It gives no reaction rates by arm and no grade distribution. It names no safety denominator beyond the cohort sizes. These are topline results from a company release, not a peer-reviewed publication.

Titre ratios are immunological comparisons, not disease endpoints. The regulatory precedent in pneumococcal conjugate vaccines is instructive. The FDA granted Capvaxive accelerated approval for pneumonia prevention on immune response measures, pending confirmatory results.

Britain moved its adult programme to a pneumococcal conjugate vaccine

Here is the point the wire coverage missed. The United Kingdom changed adult product this year. A Welsh Government circular, WHC/2025/054, records a competitive tender for supply. The contract was awarded for PCV20, sold as Prevenar 20. It states that central PPV23 supply would run down in January or February 2026. PCV20 then became available to order. The change followed advice from the Joint Committee on Vaccination and Immunisation (JCVI). In June 2023 it advised that either PCV20 or PPV23 could be used.

The UKHSA Green Book pneumococcal chapter is dated 29 July 2026. It offers “a single dose of 0.5ml of PPV23 or PCV20” to adults aged 65 and over. Clinical risk groups aged two and over are covered on the same terms. The chapter records that PCV20 became available to order in early 2026. Local PPV23 stocks should be used up first.

Two consequences follow. Both are LSDN’s own reading, not a company claim. The incumbent British adult product is the comparator VAX-31 matched on all 20 shared serotypes. Capvaxive, meanwhile, holds a Great Britain licence without a routine adult programme place. Its first authorisation was 15 May 2025, and the label text was last revised on 11 August 2026. It carries the black triangle additional monitoring symbol.

Britain’s adult pneumococcal conjugate vaccine supply now runs through one contracted product. Access for any new entrant therefore looks like a supply tender between licensed products. On this record, it is not primarily a NICE appraisal question.

Serotype 3 looms large in English hospital data

The second LSDN reading concerns which serotypes missed. Serotype 3 is no marginal strain in one large English cohort. Lansbury and colleagues studied 5,186 adults hospitalised with community-acquired pneumonia at two Nottingham teaching hospitals. The cohort ran from September 2013 to May 2023. Pneumococcal pneumonia accounted for 2,193 cases, or 42.2 per cent. The share attributed to serotype 3 rose from 13.4 per cent in 2013 to 48.8 per cent in 2023. The authors conclude that serotype 3 is now the predominant serotype in adult pneumococcal community-acquired pneumonia. They note that the rise came despite an established infant pneumococcal conjugate vaccine programme. They call that consistent with a lack of herd protection for this serotype.

The Green Book separately notes rises in serotypes 8, 12F and 9N in older age groups since 2013/14. Both serotypes where VAX-31 fell short of the prespecified bar are therefore serotypes British sources single out.

The limits of that reading deserve stating plainly. The shortfall was measured against Capvaxive, not against the British incumbent. Vaxcyte reports that both serotypes cleared the historical 0.5 threshold. A titre ratio is not a clinical outcome. The Nottingham cohort comes from one city, not a national sample. No source read for this article connects the OPUS-1 result to disease caused by serotype 3. Age itself complicates the picture, since immune responses to vaccination weaken in older adults.

The commercial frame and the filing clock

Pfizer booked $6,494 million in Prevnar family revenue for 2025, up 1 per cent operationally. It credited adult uptake in certain international markets and continued United States adult uptake. The US demand followed the CDC recommendation covering ages 50 to 64. United States revenue fell 2 per cent to $4,151 million. International revenue rose 7 per cent operationally to $2,342 million. The CDC now advises PCV15, PCV20 or PCV21 for all adults aged 50 or over without prior conjugate vaccination. That recommendation is the commercial prize a broader pneumococcal conjugate vaccine is built to win.

Vaxcyte shares opened at $87.21 on 5 October 2026, 54.4 per cent above the $56.48 close on 2 October. The gain had narrowed to about 27 per cent by 12:45 Eastern, according to stockanalysis.com. BioPharma Dive separately reported a rise of about 50 per cent in early trading. It put the company’s market value above $12 billion.

The timetable repays attention. Vaxcyte’s August filing named no year for a submission. Monday’s release names a planned Biologics License Application in the first half of 2028. OPUS-2 and OPUS-3 are due in the first half of 2027, with a manufacturing consistency study to follow. Infant Phase 2 data are expected by the end of the first half of 2027. The company held $2,507.7 million in cash, cash equivalents and investments at 30 June 2026. Its second-quarter net loss was $284.3 million.

What LSDN is watching

Three questions follow from Monday’s disclosure. The first is what OPUS-2 and OPUS-3 add on serotypes 3 and 12F. Monday’s release names no comparators for either trial. The second is what the full safety tables show once a publication or a regulatory review exposes them. The third is British, and it is a procurement question rather than a clinical one.

The current adult contract followed a competitive tender. That tender came after the JCVI advice of June 2023. The planned 2028 filing for the 31-valent pneumococcal conjugate vaccine is a United States application. Monday’s release names no British or European filing plan. Any later British application would meet a programme contracted years earlier. Readers tracking the surrounding regulatory calendar can follow our running list of FDA drug approval decisions.
