uniQure filed for regulatory approval of its Huntington’s disease gene therapy on 2 September 2026. The Huntington gene therapy submission went to two agencies. One is the US Food and Drug Administration. The other is the UK Medicines and Healthcare products Regulatory Agency. Trade coverage has described it as the first filing of a gene therapy for Huntington’s disease.

The Amsterdam-based biotech submitted a Biologics License Application to the FDA. It also filed a Marketing Authorisation Application to the MHRA. Both requests concern ifezuntirgene inilparvovec, also known as AMT-130. uniQure has asked the FDA for priority review. If granted, that would shorten the review clock to six months once the FDA’s 60-day filing review period has run.

Around 75,000 people in the United States, European Union and United Kingdom live with Huntington’s disease. No disease-modifying therapy exists. Approved medicines address symptoms only. The Huntington gene therapy submission therefore marks a possible pivot point. The field has waited decades for a candidate that could change disease trajectory.

Three-year data drove the filing

The applications rest on three-year data from the Phase I/II clinical programme. The primary US study is randomised and controlled with sham surgery. It enrolled 26 patients across low-dose, high-dose and sham arms. A parallel European study runs open-label and has enrolled 13 patients. Later cohorts added immunosuppression or targeted patients with lower striatal volumes.

uniQure compared its treated group with propensity-matched external controls. The comparator was drawn from the Enroll-HD natural history database. At three years, the high dose slowed decline on the composite Unified Huntington’s Disease Rating Scale by 75 per cent. That met the study’s prespecified primary endpoint. The mean change from baseline was -0.38 for the high dose. The matched controls posted a mean change of -1.52. The between-arm p-value was 0.003.

The high dose also slowed decline on Total Functional Capacity by 60 per cent, with a p-value of 0.033. That measure of independence in daily activities was a key secondary endpoint. Cerebrospinal fluid neurofilament light stayed below baseline at three years. That marker tracks neuronal damage. The analysis covered 29 treated patients, 17 at the high dose and 12 at the low dose. Twelve patients in each dose group had reached 36 months.

A four-year data cut is due before the end of September 2026. That readout will feed into the ongoing FDA and MHRA reviews. It will also mark the first look at durability past the three-year window.

The regulatory path narrowed and then reopened

The Huntington gene therapy submission caps a turbulent year with the FDA. In late 2025, uniQure told investors the agency had shifted position. The issue was acceptance of the Enroll-HD external control. In March 2026, the FDA told uniQure it strongly recommended a new double-blind study with a sham-surgery arm. Investigators and patient advocates called that request impractical for a fatal, low-prevalence condition.

The position changed again in June 2026, when the FDA communicated that the three-year data could support a BLA. That decision opened the accelerated approval pathway. The agency also raised a standard-of-care control as an alternative to a sham procedure in the required confirmatory trial. uniQure later told investors the two sides had aligned on a randomised standard-of-care design. Trade coverage linked the agency’s shift to leadership turnover at the FDA. uniQure said the ruling made a near-term filing possible.

AMT-130 already holds Breakthrough Therapy, Regenerative Medicine Advanced Therapy and Fast Track designations from the FDA. On 30 April 2026, uniQure reported a positive pre-submission meeting with the MHRA. That interaction set up the parallel UK filing. The company did not disclose which specific UK pathway had been agreed. It confirmed only that the meeting was constructive and pointed to a Q3 2026 submission.

The UK trade angle

The pivotal Phase I/II study runs partly at the National Hospital for Neurology and Neurosurgery in London. That site is part of University College London Hospitals NHS Foundation Trust. Sarah Tabrizi is Professor of Clinical Neurology at UCL. She is also Honorary Consultant Neurologist at the National Hospital for Neurology and Neurosurgery, UCLH. She is lead scientific advisor to the study. She described the three-year data as the most convincing in the field to date. That comment came in a statement issued with uniQure’s results announcement. Edward Wild, Consultant Neurologist at UCLH, has flagged both the promise and a delivery constraint.

Speaking at the Advanced Therapies conference in London in March 2026, Wild said the bottleneck may come from surgical delivery. Pharmaceutical Technology reported the comments. He noted that MRI-guided stereotactic surgery is not available in every country. AMT-130 requires a single MRI-guided neurosurgical procedure into the striatum.

The MHRA route places the UK in step with the FDA rather than behind it. Should the MHRA accept and act on the MAA, uptake still depends on two further steps. NICE would need to appraise the therapy within its marketing authorisation. The NHS would then need to commission the specialist neurosurgery.

Safety and the trial history

Safety history is not immaterial. In August 2022, uniQure paused enrolment in the high-dose cohort. The pause followed three suspected unexpected serious adverse reactions. The company reported that they involved localised inflammatory responses and related symptoms. Two events occurred at a single European site and one in the United States. uniQure said patients recovered fully or substantially. It resumed the programme with modified monitoring and treatment protocols.

uniQure reported no new drug-related serious adverse events since December 2022, as at the June 2025 data cutoff. Most subsequent adverse events have related to the surgical procedure itself. The company describes tolerability as manageable. Independent reviewers should still scrutinise the full safety dataset when it appears in labelling. The neurosurgical procedure remains an intrinsic part of the risk profile. It is not a factor that can be reduced by patient selection alone.

Market reaction to the Huntington gene therapy submission

uniQure shares closed down 4.95 per cent at $45.49 on the day of the Huntington gene therapy submission. The ticker is QURE on the Nasdaq. Benzinga Pro published the price. The move suggests investors are waiting for FDA acceptance rather than the filing itself. Filing acceptance is not a foregone conclusion. It also does not lock in a specific review clock.

William Blair analyst Sami Corwin resumed coverage with an Outperform rating and a $73 fair value estimate. Corwin called BLA acceptance an incremental catalyst. William Blair suggested AMT-130 could be approved in both the US and UK in 2027. Corwin also estimates peak global sales of $3.2 billion in 2035. Leerink Partners analyst Joseph Schwartz took a similar view in a note reported by BioPharma Dive. He said avoiding a sham procedure should make the confirmatory study more practical to enrol. Both notes are analyst views rather than company guidance.

What to watch next

Three near-term signals matter for the field. Filing acceptance by the FDA and MHRA is the first. The four-year data cut, due by the end of September 2026, is the second. A confirmatory trial design that fits FDA and MHRA guidance is the third.

If the Huntington gene therapy submission is accepted and cleared, delivery capacity will decide reach. MRI-guided striatal surgery runs in a small number of specialist centres. The UK trial neurosurgeries were performed in Cardiff at the University Hospital of Wales. Building UK capacity would fall to the NHS if an approval follows the current timeline.

The uniQure Huntington gene therapy submission also carries a wider signal. It reopens accelerated approval as a route for gene therapies in neurodegenerative disease. Rare-disease sponsors have watched this file closely. Recent LSDN coverage of the Genglycos gene therapy approval showed how far a surrogate-endpoint route can go. That approval addressed an ultra-rare metabolic disease. This filing tests whether the same logic holds for a neurological condition with an external comparator. The UK response arrives during a fortnight of MHRA activity. It sits alongside the recent avacopan UK suspension. Together the two cases will show how the MHRA weighs access against evidentiary standards on very different files.
