Icotrokinra NHS access has no published timetable. As of 14 October 2026, LSDN searches found no UK marketing authorisation for icotrokinra. The drug is approved as Icotyde in the US and EU. The European Commission authorisation was announced on 21 September 2026. Johnson & Johnson (J&J) presented two-year ICONIC-LEAD data on 9 October 2026. The UK steps that would follow are visible, but none has a public date.

How Icotrokinra Was Approved in the US and EU

The FDA lists Icotyde (icotrokinra) among its 2026 novel drug approvals, with an approval date of 17 March 2026. J&J announced the approval on 18 March. The indication is moderate to severe plaque psoriasis. It covers adults and adolescents aged 12 and over who weigh at least 40 kg. Patients must be candidates for systemic therapy or phototherapy.

In Europe, the CHMP recommended authorisation at its meeting of 20 to 23 July 2026. The EMA described it as the first oral medicine targeting the interleukin-23 receptor. J&J then announced European Commission approval on 21 September 2026. The European indication covers adults and adolescents aged 12 and over who weigh at least 40 kg.

The EMA said each member state decides on pricing and reimbursement separately. J&J’s European release gives no price, reimbursement decision or launch date.

Icotrokinra NHS Access: Where the UK Process Stands

Three steps stand between the drug and NHS patients. The MHRA must authorise it for Great Britain. NICE must appraise it. The NHS must then fund it. Icotrokinra NHS access also depends on the sponsor’s choices. J&J’s European release says it is working with stakeholders across Europe to support introduction. It names no UK step.

MHRA: No Authorisation Record Found

LSDN searched the emc medicines database for icotrokinra on 14 October 2026. The search returned no results. The GOV.UK search page could not be read by automated tools. LSDN therefore cannot rule out a very recent decision.

NICE’s draft scope said the drug “does not currently have a marketing authorisation in the UK” for this use. The draft scope is dated November 2025. NICE’s timeline places the scoping consultation between 10 November and 8 December 2025.

The MHRA has a route that relies on other regulators’ decisions. Its International Recognition Procedure lists the FDA and the EMA among its reference regulators. Recognition A runs for 60 days. Recognition B runs for 110 days.

MHRA guidance says a first-in-class new active substance is automatically placed on Recognition B. LSDN has seen no MHRA statement on which route, if any, applies to icotrokinra. LSDN also found no public record of a UK filing.

NICE: Appraisal in Progress, No Publication Date

NICE lists an appraisal of icotrokinra, project ID6579. The remit is moderate to severe plaque psoriasis in people aged 12 and over. It is a single technology appraisal, the standard process type. Its status is “in progress”, and the latest entry reads “scoping commencing” on 10 November 2025.

The page lists no expected publication date. It lists no final scope or committee meeting date either. The draft scope names the treatments NICE may compare with icotrokinra. For adults with severe disease, these include guselkumab, risankizumab, bimekizumab, ixekizumab and deucravacitinib. Earlier options include methotrexate, ciclosporin and acitretin.

Funding follows a NICE recommendation. NICE states that the NHS must fund a recommended treatment within three months of publication. Guidance can set a different period. The icotrokinra NHS timeline therefore depends first on the MHRA and NICE.

What the Trial Data Would Have to Show

The approvals rest on the ICONIC programme. The EMA opinion considered four phase 3 trials with about 2,500 patients. In three studies, 50 to 57 per cent of patients on icotrokinra reached PASI 90 at week 16. Placebo rates were 1 to 4 per cent.

The EMA also reported clear or almost clear skin (IGA 0 or 1) in 65 to 71 per cent. The placebo range was 8 to 11 per cent. The fourth study covered scalp, genital, hand and foot disease.

ICONIC-LEAD enrolled 684 participants. Of these, 456 received icotrokinra and 228 received placebo. It included 66 adolescents. The four registered trials are ICONIC-LEAD (NCT06095115), ICONIC-TOTAL (NCT06095102), ICONIC-ADVANCE 1 (NCT06143878) and ICONIC-ADVANCE 2 (NCT06220604). LSDN could not read ClinicalTrials.gov, so it states no registry status. These trials would form the evidence base for any icotrokinra NHS submission.

ICONIC-ADVANCE 1 and 2 compared icotrokinra with placebo and deucravacitinib. J&J describes these as head-to-head superiority studies. It says about 70 per cent reached IGA 0 or 1 at week 16. It says 55 per cent reached PASI 90. The releases LSDN read give no deucravacitinib-arm figures. NICE lists deucravacitinib as a comparator, so those figures will matter to the appraisal.

On 9 October 2026, J&J presented two-year ICONIC-LEAD data at Fall Clinical Dermatology 2026. It reported PASI 90 responses of at least 72 per cent from week 64 to week 112. IGA 0 or 1 responses were at least 70 per cent. The release gives no placebo comparison for that period. J&J also reports that 85 per cent of week 24 withdrawals regained PASI 90 within 24 weeks of retreatment.

Mark Lebwohl commented on the data in the J&J release. He is professor and chair emeritus of dermatology at the Icahn School of Medicine at Mount Sinai. J&J discloses that he is a paid consultant. He said: “As plaque psoriasis is a chronic lifelong disease, durable results are essential in therapy selection.”

Safety Findings and Label Cautions

The current US label shows the adverse reaction rates through week 16. DailyMed lists its revision date as March 2026. In 1,296 patients on icotrokinra and 568 on placebo, headache occurred in 4.1 per cent and 3.3 per cent. Nausea occurred in 1.2 per cent and 0.5 per cent.

Cough occurred in 1.2 per cent and 0.2 per cent. Fungal infection occurred in 1.1 per cent and none of the placebo group. Fatigue occurred in 1.0 per cent and 0.5 per cent. The label lists no contraindications.

The label’s warnings cover infections, tuberculosis and immunisations. These are the points NHS prescribers would weigh if icotrokinra NHS use began. The label advises considering tuberculosis evaluation before treatment and monitoring for active disease during and after it. It says live vaccines should be avoided. The label says available pregnancy data are insufficient to evaluate drug-associated risk.

J&J also reports that adverse reaction rates through week 16 were within 1.1 per cent of placebo. It reports no new safety signals through week 52, and through week 112 in the two-year poster data. The release gives no per-arm safety tables for the two-year period.

What Would Settle the Icotrokinra NHS Question, and When

Three public records would answer the question. The first is an MHRA authorisation entry. The second is a NICE timetable or draft guidance. The third is a J&J statement on a UK filing. None had appeared when LSDN checked on 14 October 2026.

No source gives dates for these steps, so LSDN offers no forecast. NICE’s timeline will be the clearest marker for NHS access. Readers tracking US decisions can follow the FDA drug approval decisions tracker. LSDN also examined NICE scoping for atezolizumab in dMMR colon cancer, another US-approved drug awaiting UK steps.
